I, the undersigned, acknowledge that as an employee of Peak 7 Adventures, I am a Mandatory Reporter under Washington State Law (RCW 26.44.030). By signing this form, I confirm that I have completed the mandatory reporter training provided by The Alliance for Professional Development, Training and Caregiver Excellence and understand the following:

Duty to Report: I understand that I am legally required to make a report to the Department of Children, Youth, and Families (DCYF) or law enforcement if I have reasonable cause to believe that a child has suffered abuse or neglect.

Timing of Report: I understand that a report must be made at the first opportunity, but no later than 48 hours after I have reasonable cause to believe the abuse or neglect has occurred.

Recognize, Record, Report: I have been trained to properly Recognize the signs of abuse and neglect, Record objective observations, and Report to the proper authorities (1-866-ENDHARM).

Poverty vs. Neglect: I understand the distinction between families in need of community support (poverty) and actual child neglect. I am committed to using the resources provided in training to help support families while prioritizing the safety of the child.

Failure to Report: I understand that a willful failure to report is a gross misdemeanor in the state of Washington and may lead to disciplinary action by Peak 7, up to and including termination.

Immunity & Protection: I understand that Washington law provides immunity from civil or criminal liability for reports made in good faith.

Confidentiality: I understand that while DCYF protects the identity of reporters to the extent possible, my identity may be disclosed under certain legal circumstances (such as a court order).
